Endoscopic Repair of Septal Perforation

Indications:

  • Anterior septal perforation
  • Symptoms of nasal crusting, bleeding, or whistling
  • Perforation diameter of less than 15 mm

Highlights:

  • Edges of perforatioin are freshened
  • Elevation of bilateral septal mucosal flaps
  • Flaps sutured anteriorly for 2-layer closure

Keep in mind:

  • Surgical repair does not have a high success rate for perforations larger than 15 mm
  • Although surgery may close a symptomatic anterior perforation, rotation of mucosal flaps may result in a new asymptomatic posterior perforation.
