Endoscopic Repair of Septal Perforation

Indications:
- Anterior septal perforation
- Symptoms of nasal crusting, bleeding, or whistling
- Perforation diameter of less than 15 mm
Highlights:
- Edges of perforatioin are freshened
- Elevation of bilateral septal mucosal flaps
- Flaps sutured anteriorly for 2-layer closure
Keep in mind:
- Surgical repair does not have a high success rate for perforations larger than 15 mm
- Although surgery may close a symptomatic anterior perforation, rotation of mucosal flaps may result in a new asymptomatic posterior perforation.
